Though this form does not specifically mention housing, it is used for requesting adaptations within the workplace and/or housing.
With the COVID-19 pandemic continuing to disrupt our daily lives, we continue to focus on the safety and well-being of our Guests and Cast Members. At this time, Disneyland and Walt Disney World Resorts remain closed until further notice.
Due to this, please note that your request will be placed on hold, and you will only hear back regarding your request once we are fully operational again.
When completed, these documents should be sent by fax to the Health Services department - 407-938-4105 (Walt Disney World Resort - Florida) / 714-781-4249 (Disneyland Resort - California) or by email to DPECP.Medical.Accommodations@disney.com. Operating participants should reach out to their company’s Human Resources department.
For those requesting an Emotional Support or Service Animal, please follow the medical accommodation process. Once you are approved, you will receive communication from the Housing team in regards to next steps and details surrounding your Housing placement.
Please keep in mind the housing team does not have access or the ability to review or discuss accommodations with you, your doctor, or your family members. You may contact Employee Relations at 407‐938‐1511 with questions.
