Step by Step Guide for Completing your I9 Form
To successfully complete your I9 Form, keep this guide open and use it as a reference as you work through each section. Before clicking Continue on any page, double-check your responses against the guide to ensure all information is entered correctly and to help avoid submission errors.
Note: If you have already completed Section 1 and Section 2 of your I9 Form please scroll down and read the Next Steps section of this article.
Purpose of the I9 Form
The I9 Form is used to verify your identity and eligibility to work in the U.S. regardless of citizenship status. Everly U.S. employer must collect this form from new hires and returning employees. The I9 Form is not a tax document.
You will receive a link to the I-9 Service Center from The Walt Disney Company I-9 Service Center (DoNotReply@Fragomen.com) at least four weeks prior to your arrival. Your I-9 Form consists of two sections: Section 1 and Section 2.
It is critical that both sections are completed accurately and without errors, as incomplete or incorrect information may delay your onboarding process. Both sections must be fully completed no later than 3 weeks prior to your arrival.
Before You Start
Have the following information ready before you log into the I9 Service Center and start on your I9 Form.
1. Your I-9 Documents
Review the list of acceptable documents on the USCIS website: https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents
You must provide either:
- One (1) document from List A, OR
- One (1) document from List B and one (1) document from List C
All documents must be original and unexpired.
Important: IF you choose to use your Passport, make sure it is signed by you on the “Signature of Bearer” line.
2. Your Authorized Representative's Information
You will need your Authorized Representative's:
- Full name
- Email address
Your Authorized Representative must:
- Be 18 years of age or older
- Be someone you trust
- Be available to meet with you in person
Examples include a parent, relative, friend, professor, advisor, or another trusted adult.
Your Authorized Representative will be responsible for physically reviewing your original documents and completing Section 2 of your Form I-9. The system will prompt you to assign an Authorized Representative after you fill out Section 1 of your I9 Form.
I9 Service Center Log In
You will receive a link to the I-9 Service Center from The Walt Disney Company I-9 Service Center (DoNotReply@Fragomen.com) at least four weeks prior to your arrival.
Filling Out Your I9 Form
Your I9 Form will have two sections: Section 1 and Section 2. Please make sure to read everything carefully and double check your information is accurate before clicking Continue to go the next page. DO NOT use autofill.
Section 1
Section 1 must be completed by you.
Step 1: Enter Your Name
Type your:
- Last Name (If you have two last names listed on your documents, please place BOTH on the Last Name box)
- First Name (Given Name)
- Middle Initial (only enter if it is listed on your documents)
- Other Last Names Used (this is to be used ONLY IF you have LEGALLY changed your last name)
Tip: Use your legal name exactly as it appears on your government-issued documents.
Step 2: Enter Your Address
Provide your current residential address.
Include:
- Street Number and Street Name
- Apartment Number (if applicable)
- City
- State
- 5 digit ZIP Code
Step 3: Enter Your Date of Birth
Use the format:
MM/DD/YYYY
Step 4: Enter Your Social Security Number
If your employer participates in E-Verify, you must provide your Social Security Number.
Example:
123-45-6789
Important: Double-check every number before continuing.
Step 5: Enter Your Contact Information (optional)
Provide:
- Email Address
- Phone Number
Step 6: Select Your Citizenship or Immigration Status
Choose ONE of the following boxes:
Option 1
☐ A citizen of the United States
Check this box if you are a U.S. citizen.
Option 2
☐ A noncitizen national of the United States
Most participants will not select this option.
Option 3
☐ A lawful permanent resident
Choose this if you have a Permanent Resident Card (Green Card).
You must provide your:
- USCIS Number or A-Number
Option 4
☐ A noncitizen authorized to work
Choose this if you are working based on a visa or other employment authorization.
You may need to enter:
- Expiration date of work authorization
- USCIS A-Number
- Form I-94 Admission Number
- Foreign Passport Number and Country of Issuance
Important: If you are unsure which box to select, review your immigration documents carefully before completing the form.
Step 7: Review Everything
Before signing:
- Check that your name is spelled correctly
- Verify your address is complete
- Confirm your date of birth is correct
- Make sure your Social Security Number is accurate
- Verify you selected the correct citizenship/immigration status
Even a small typo can result in a correction request later.
Step 8: Sign and Date the Form
Read the employee attestation statement then sign your name exactly as you entered it at the beginning of Section 1.
By signing, you are confirming that all information you entered is true and accurate.
Step 9: Upload Your Documents
The documents you selected at the beginning of the I9 process, must be uploaded to the I9 Service Center
- Select the correct document type for your document. Below are the most common documents that are used for the I9 Form and their correct document types on the I9 Service Center:

- Make sure the picture is clear and all 4 sides of the documents are showing (no glares or cut-offs).
Step 10: Assign Your Authorized Representative
Enter your Authorized Representatives’ full name and email address.
As a reminder, your Authorized Representative must be someone of trust over the age of 18 that you can meet with personally.
Once you assign your Authorized Representative, they will receive an email with their own log in credentials to access Section 2 of your I9 Form.
Section 2
Please share this information with your Authorized Representative.
The purpose of an Authorized Representative is to physically examine your work authorization documents on behalf of the employer when the employer cannot physically examine the documents.
Before Your Authorized Representative Starts on Your I9 Form
- Meet in person with your Authorized Representative
- Bring the same documents you uploaded to the I9 Service Center and hand your document to your Authorized Representative.
- Your Authorized Representative will review your documents and make sure they are original, appear genuine and belong to you.
Filling out Section 2
Please have your Authorized Representative carefully read the instructions on the screen and accurately fill out Section 2 of your I9 Form.
Step 1: Log Into the I9 Service Center
Your Authorized Representative will receive an email with their own log-in credentials. Once they log in, they will open the I9 and read the instructions carefully.
Step 2: Select Document Type
Your Authorized Representative will select the same document types you selected when you uploaded your document to the I9 Service Center.
Below are the most common documents that are used for the I9 Form and their correct document types on the I9 Service Center:

Only select the document type with “Receipt:” at the beginning of the title if you are uploading a receipt stating that you requested a lost, damaged or stolen document.
Example, if you presented your driver’s license and Social Security number card, your Authorized Representative would select the document types of State Driver’s License and U.S. Social Security account number card that is unrestricted.
Step 3: Enter Document Information
Make sure you fill out all the fields are accurately filled out before continuing.
- Issuing Authority
This is the specific government agency, state, or organization that officially created and gave you the document.
Here are some examples:
- For a U.S. Passport - U.S. Department of State
- For a Birth Certificate - State of Florida
- For a Social Security Card - Social Security Administration
- Document Number
Enter the document identification number exactly as it appears on the document.
- Expiration Date (if applicable)
Enter the expiration date shown on the document.
If no expiration date exists, please leave field blank.
Important: The document information entered in Section 2 must belong to the participant NOT the Authorized Representative
Step 4: Enter Start Date
The start date will be the Friday of your arrival week, NOT your arrival date.
Step 5: Sign and Date Section 2
It is normal that the employer information appears in Section 2. Read and check the attestation box and type your full name.
Next Steps
In the days leading up to your arrival date, The Disney Global HR Operations (GHRO) Onboarding team will be auditing all participant I-9 Forms to ensure they are accurate and compliant. Due to the high volume of forms being reviewed, any required corrections may not be sent all at once. We appreciate your patience throughout this process.
If a correction is needed GHRO will notify both you and your Authorized Representative via email from DoNotReply@Fragomen.com. It is important that you stay in contact with your Authorized Representative and regularly monitor your email for updates.
If you continue to receive daily Action Required emails, log in to your I-9 Service Center account and review the following:
- The document types listed on your Upload Documents page match the documents entered in Section 2 of your I-9 Form.
- The documents you uploaded are the same documents you presented to your Authorized Representative.
- Carefully review the information entered on your I9 Form making sure it’s accurate and matches the documents you presented and uploaded.
Reminder: You can view your I-9 Form at any time by selecting View I-9 from your I-9 Service Center dashboard.
